I am so happy that you want to carry my quilt patterns in your shop! We just have a couple of hoops to jump through. If your shop is located in Missouri, I will need you to fill out and return Sales Tax Exemption Certificate 149.
I have filled out all of my information and just need you to:
Click the button above and print out the PDF (the print button on the form doesn’t work, just push print on your computer)
Fill out the top portion
Enter your tax ID number in the third section
Sign and date it at the bottom
Email me a digital copy of the form (scanned or a clear photo of the entire document). If you would prefer to mail me the physical form, please email me to let me know it is on the way (my physical address is on the form).
I want to stay tax compliant, so I will wait until I have the form before shipping.
